Outcome: To improve and maintain the health and well-being of Wyandotte County residents.

Indicators:

  1. Increase the number of individuals referred to mental health services.
  2. Increase the number of Individuals that gain control of blood sugar and reduce dependency on medications.
  3. Increase the number of individuals that improve control of blood pressure.
  4. Increase the number of individuals that achieve a statistically meaningful reduction in their Body Mass Index (BMI).
  5. Increase the number of individuals served through coordination of all facets of health care within the community healthcare service system.
  6. Increase access to healthcare.
  7. Increase the number of individuals connecting to health insurance programs.
  8. Increase the number of people eating healthy food.
  9. Increase the number of people using wellness programs.
